Facility Use Application // Memorial or Funeral (Anaheim)

Crosspointe has a strong desire to serve the families of our church and community. The purpose of this form is to maintain strong communication and clear expectations between the church and the parties organizing this service. Members of Crosspointe will receive a 25% discount on the facility usage fees listed below. This does not include the officiant fee. In addition, Active Crosspointe members can contact Crosspointe Events Coordinator Cindy Berger at cindy@crosspointe.com regarding any special circumstances.

Please put the reasonable time range that you would need the facility including set up and tear down. For example, if your service is from 6-8pm, you might say 4pm- 9pm.

Date

Typically, funerals usually involve the assistance of mortuary staff. Memorial services are hosted by family and friends of the deceased.

*The facility useage fees listed below include janitorial services, set up, tear down and one audio engineer.*
You may select multiple if needed, and the main lobby is included in each option.

**Additional Personnel i.e. video engineer, second audio engineer will be an additional charged at an hourly rate of $50 P/H**

A $300 honorarium will be required in addition to the facility usage fee. Payable directly to the officiant.

Please choose at least two pastors to serve as your officiant. This does not guarantee their placement, but we will do our best to schedule one of the requested pastors.

Any video/ images used must be in .mp4 or .mov format and delivered to the audio engineer on a USB stick or via link sent 24 hours before the service.

We suggest any video be limited to 5 minutes or less.

Please keep in mind that the Hope Center capacity with tables is 80. If answer was "no", write "N/A".

Two checks should be written and delivered to the officiant the day of the event. One check is payable to "Crosspointe Church" for the facility usage fee. The second is payable to the officiant in the amount of $300. Should you prefer to pay the facility usage fees with a card, you can do so by contacting the church office prior to the date of the service. (714) 777-3440

All non Crosspointe applicants using Crosspointe Church facilities shall provide a Certificate of Liability Insurance with limits of at least $1,000,000 per usage, and shall name Crosspointe Church as the additional insured. The Certificate of Liability Insurance shall identify the date(s) of the event(s) at Crosspointe Church. All costs associated with the Certificate of Liability Insurance shall be borne by the applicant. The Certificate of Liability Insurance must be submitted along with the initial deposit.

You have thoroughly read the Facility Rental/Service Application. You are willing to abide by the guidelines established therein and will make every effort to see that they are observed by those involved in our event. Should Crosspointe incur any damages as a result of your event and/ or it's attendees, you agree to take full responsibility and pay all costs associated with repairing or replacing the damaged property. Also, you understand that Crosspointe is not responsible for any lost or stolen property belonging to your attendees nor is Crosspointe responsible for any injuries or accidents during your event.
